• Attend scheduled appointments and notify us in advance if you need to cancel or reschedule.

3. Medical Consultation

All patients must undergo a medical consultation before receiving an IV infusion. This consultation may include:

• Medical consultation with a Registered Nurse
• Discussion of potential risks and benefits

4. Risks and Side Effects

IV infusions carry potential risks and side effects, including but not limited to:

• Allergic reactions
• Irritation or bruising at the injection site
• Vein irritation

You acknowledge that you have discussed these risks with your healthcare provider and understand them.

5. Payment and Insurance

• Payment is due at the time of service unless prior arrangements have been made.
• We accept various forms of payment, including credit/debit cards, cash and afterpay.
• If applicable, we will provide you with a receipt for insurance reimbursement. However, it is your responsibility to verify coverage with your insurance provider.

6. Cancellations and No-Shows

We request a minimum of 48 hours notice for cancellations or rescheduling of appointments. Failure to do so may result in a cancellation fee 20% of total services cost at time of booking

7. Privacy Policy

Your personal health information is protected in accordance with our Privacy Policy. We are committed to safeguarding your information and only use it for purposes related to your care.

8. Limitation of Liability

To the fullest extent permitted by law, Infuse Me IV and its employees shall not be liable for any direct, indirect, incidental, or consequential damages resulting from your use of our services.
